- the file cache handler initialising the WordPress filesystem on every request at plugins_loaded - on hosts where WordPress cannot write files directly (FTP/SSH credential hosts) this echoed the WordPress connection-details form into every page load, WP-CLI command, cron run and REST request. The filesystem is now initialised only when a cache method is actually used, and the credentials prompt has been removed entirely (if credential-less filesystem access is unavailable, file caching is simply reported as unavailable and callers fall back to other cache storage)
- a potential fatal error in the cache handler when the filesystem could not be initialised (cache methods now fail gracefully instead of calling methods on a null filesystem object)
- cache expiry never being detected on read - the expiry timestamp comparison compared a number against a date string, so expired cache files were still served until the cleanup cron removed them
